Job Summary:

We are seeking a dedicated and compassionate PRN Physical Therapist to join our team at Emerald Care Center Claremore, a leading skilled nursing facility committed to providing exceptional care to our residents. As a Physical Therapist, you will play a crucial role in assessing, planning, and implementing personalized rehabilitation programs designed to enhance the mobility, strength, and overall quality of life for our residents.

Responsibilities:

  • Assessment and Evaluation: Conduct comprehensive evaluations of residents to assess their physical abilities, limitations, and functional needs. Develop individualized treatment plans based on assessment findings and residents’ goals.
  • Treatment Planning and Implementation: Design and implement therapeutic exercise programs to improve strength, mobility, balance, and coordination. Utilize a variety of therapeutic techniques and modalities to address specific conditions and promote recovery. Educate residents and their families about exercises, techniques, and strategies for improving mobility and managing conditions.
  • Documentation and Reporting: Maintain accurate and detailed documentation of assessments, treatment plans, progress notes, and discharge summaries. Monitor and report on residents’ progress, adjusting treatment plans as needed to ensure optimal outcomes.
  • Collaboration and Communication: Work closely with interdisciplinary teams, including physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and ensure a holistic approach to treatment. Communicate effectively with residents and their families to provide education, support, and updates on treatment progress.
  • Compliance and Safety: Adhere to facility policies, procedures, and regulatory requirements related to physical therapy practices. Ensure a safe and supportive environment for residents during therapy sessions, adhering to infection control and safety protocols.
  • Professional Development: Stay current with advancements in physical therapy techniques, best practices, and relevant research. Participate in continuing education opportunities and professional development activities to enhance skills and knowledge.

Qualifications:

  • Education: Doctoral or Master’s degree in Physical Therapy from an accredited program.
  • Licensure: Current state licensure as a Physical Therapist.
  • Experience: Previous experience in a skilled nursing facility or similar healthcare setting is preferred but not required.
