Job Title: Pharmacist

Position Summary
At AXIS Rx, we believe access to care includes access to the medications that support whole-person care and well-being. The Pharmacist plays a critical role in ensuring safe, effective, and appropriate medication therapy as part of an integrated care model.
Working closely with pharmacy technicians, providers, and interdisciplinary care teams, this position is responsible for overseeing medication management, clinical review, dispensing accuracy, and client consultation in a fast-paced, customer-centric environment.
Key Responsibilities
• Review, verify, and dispense medications accurately and in compliance with all regulations
• Provide clinical oversight of prescription processing and pharmacy operations
• Counsel clients on medication use, side effects, interactions, and adherence
• Collaborate with providers and care teams to optimize medication therapy and support treatment goals
• Monitor for drug interactions, contraindications, and therapeutic appropriateness
• Supervise and support pharmacy technicians and support staff
• Ensure compliance with all regulatory, safety, and organizational standards
• Support medication coordination within an integrated care model
• Address insurance, prior authorization, and medication access issues as needed
• Maintain accurate and timely documentation in pharmacy and health record systems
• Promote a culture of safety, quality, and continuous improvement
• Provide respectful, compassionate service to clients and care partners
• Maintain confidentiality of client health information at all times
Qualifications
• Doctor of Pharmacy (PharmD) or equivalent degree required
• Active pharmacist license in good standing in the applicable state
• Immunization certification preferred (if applicable)
• Previous experience in pharmacy practice or healthcare setting preferred

Preferred Experience
• Experience in outpatient, behavioral health, or integrated care settings
• Familiarity with medication-assisted treatment (MAT) or behavioral health medications
• Experience working with electronic health records (EHR) and pharmacy systems
Work Environment & Expectations
This role operates in a clinical pharmacy setting and requires standing for extended periods, overseeing workflow, and managing multiple priorities throughout the day. Pharmacists are expected to lead by example and contribute to a culture of collaboration, accountability, and respect.
