Under immediate to general supervision, as part of a Wraparound Team, provides targeted support services to provide a system of care that is child-centered with the needs of the child and family dictating the types and mix of services provided to assist in preventing an out-of-home and out-of-school placement. This position’s general and immediate supervision is to be determined. The Family Support Provider will ensure that service provision documentation are conducted to the highest standards of fidelity of the Wraparound Care Model.
EDUCATION AND EXPERIENCE:
· High School Diploma or equivalent. Experience in seeking and accessing state and community resources and a willingness to help families access resources.
· Experience raising a child with behavioral/emotional issues.
· Experience accessing services for children with behavioral/emotional issues and navigating the child serving system.
· An expectation of core competencies in relation to individuals with co-occurring disorders.
KNOWLEDGE AND SKILLS:
· Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.
· Ability to establish working relationships with clinical, administrative, and support staff in order to develop professionalism and coordination within the team.
· Able to establish and maintain an effective working relationship with fellow employees, clients and the public.
· Ability to interact with staff, clients and administration and the community both in person and on the telephone.
· Ability to maintain strict confidentiality guidelines for interactions regarding client information.
· Ability to work well under pressure meeting deadlines with efficiency and consistency.
· Such specialized training and/or certification in mental health services or related field as may be required in order to provide adjunctive services, i.e., Systems of Care/Wraparound training.
· Personality and mature presentation of oneself demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.
JOB DUTIES AND RESPONSIBILITIES:
· Assist families in developing and recruiting informal supports to be a part of the Family Team meetings.
· Complete (or co-complete) Strengths Needs Cultural Discovery (SNCD) assessments, Functional Assessments/Crisis Plans, Safety Plans, and Care Plans/Updates.
· Provide support for families with the eventual goal of the family utilizing their natural supports that is client-centered and specific to the culture of the family.
