Position Summary:

The Skills Development Professional (SDP) is an integral member of the Clinical Services team, this position provides rehabilitation, wellness, and case management services directly to clients as outlined in the client’s individual treatment plan. Under the immediate supervision of the Integrated Team Manager, this role ensures that services occur at the required frequencies, quality standards are met and proficiently completes documentation of services.

This position requires strict adherence to ethical and professional boundaries, a strong commitment to confidentiality, and the ability to collaborate effectively with internal team members and external community partners. Emphasis is placed on person-centered care, strong communication skills, accurate and timely documentation, and meeting billable service and certification requirements.

EDUCATION, QUALIFICATIONS, AND EXPERIENCE:

• Case Management II certification; or

• High school diploma or equivalent and a total of 36 months of direct, documented experience working with persons who live with mental illness and/or substance abuse issues; or

• 60 college credit hours in any field and 12 months of behavioral health experience; or

• Bachelor's or Master's degree in any field with 6 months of Behavioral Health experience; or

• Bachelor's or Master's Degree in a Behavioral Health field

• Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.

• Ability to establish working relationships with clinical, administrative and support staff in order to develop professionalism and coordination within the team.

• Ability to establish and maintain an effective working relationship with fellow employees, clients and community partners.
